Randomized controlled trial of Surveilled real-time, home-based videoconferencing cardiac telerehabilitation versus center-based cardiac rehabilitation using the Wavy Meet Platform
The Lets’Telerehab study aims to evaluate a new home-based cardiac rehabilitation programme delivered through secure video consultations. Using the Wavy Meet platform, developed by the University of Luxembourg in collaboration with clinical experts, participants can complete supervised rehabilitation sessions from home while their heart activity is monitored in real time by their therapist. The study will compare this innovative tele-rehabilitation approach with standard hospital-based rehabilitation and with conventional home-based rehabilitation where heart data are reviewed after each session. Researchers will assess whether this new approach is feasible and whether it can improve physical fitness, quality of life, and access to cardiac rehabilitation for people recovering from heart disease.

Who can participate?
Eligible participants
- Patients older than 18 years and suffer from cardiovascular disease (coronary artery disease, valve disease) with a low to intermediate risk of Event according to the American Association for Cardiovascular and Pulmonary Rehabilitation (AACVPR).
- Eligible patients must be on optimal medical treatment and be clinically stable for at least 6 weeks and be able to perform exercise, have access to the IT- hardware needed to install the -Wavy Meet platform,
- Have enough space at home to safely perform the exercises
- A support person present at home during the rehabilitation sessions (for safety reasons).
Exclusion criteria
Patients with a contraindication for cardiac rehabilitation (unstable angina, advanced heart failure, high blood pressure, left ventricular outflow tract obstruction, grade 2 and 3 AV block, myocarditis, active pericarditis, severe valvular disease, ventricular arrhythmias, aortic dissection, acute thrombophlebitis, pulmonary or systemic embolism, severe psychological disorders or severe mobility limitations), have a severe cognitive disorder or do not have a support person at home during the rehabilitation sessions

What will participants do?
01.
Baseline Assessment at CHL
The study team reviews the patient’s medical history, assesses physical fitness, and measures grip strength. Patients also complete questionnaires on quality of life, physical activity, and expectations for cardiac rehabilitation.
02.
Random Assignment to a Programme
Patients are randomly assigned to one of three rehabilitation programmes: standard hospital-based cardiac rehabilitation, home-based rehabilitation by videoconference with heart rate data reviewed after each session, or home-based rehabilitation using the Wavy Meet platform with real-time video supervision and live heart rate monitoring.
03.
8-Week Rehabilitation Programme
Patients take part in three supervised sessions per week, each lasting approximately 50 minutes and including a warm-up, exercise training, and a cool-down. A heart rate monitor is worn during every session.
04.
Set Up for Home-Based Programmes
Patients assigned to a home-based programme install the videoconferencing software and use the exercise equipment provided by the study team.
05.
Final Assessment at CHL
After completing the 8-week programme, patients return to CHL for the same evaluations performed at baseline, plus an additional questionnaire about their experience with the rehabilitation programme.
